I explain what makes a web form HIPAA safe and how to check your own forms this week. I also share what I am doing on my servers and where I can help.
What it is
A HIPAA-safe web form protects health information in transit, in storage, and through access controls. Protected health information, or PHI, is any health detail linked to a person, such as symptoms with a name. HIPAA specifics depend on your situation, so confirm details with your attorney or compliance advisor.
Why it matters
Forms can expose PHI through email, logs, plugins, or third party tools. Good controls reduce risk, support compliance, and maintain patient trust. The basics are simple and you can check most of them in minutes.
Secure transport
Use HTTPS with TLS 1.2 or higher on every page of your site, not only the form page. Turn on HTTP Strict Transport Security so browsers always use HTTPS. Do not send PHI by plain email. Send a notice without PHI and view the submission in a secure portal instead.
Secure storage
Store submissions in a database, not in inboxes. Encrypt data at rest and protect the encryption keys. Keep logs and error reports free of PHI. Keep only what you need and define a routine to delete old submissions.
Access controls
Give each staff member their own account, with the least access needed to do their job. Require multi factor authentication for admin and form data views. Log access to PHI and review it. Back up encrypted and limit who can restore.
Vendors and email
If a third party touches PHI, get a business associate agreement in place. Confirm where the data is stored, how it is encrypted, and how you can delete it. Use a CAPTCHA to reduce spam and form abuse. Avoid pasting PHI into chat tools or ticket systems that are not approved.
Test and monitor
Check that every form page shows https in the address bar. Submit a test and make sure no PHI lands in any email, log, or analytics report. Rate limit form endpoints and use a web application firewall to cut common attacks.
What I am doing on my systems
I prefer TLS 1.3 where possible, with HSTS and no mixed content. I block emails that include PHI and store submissions in encrypted databases. Admin access uses multi factor authentication and least privilege. I add web application firewall rules, rate limits, and routine permission reviews.
How I can help
If you want support, I can assess and harden your forms under Security and Compliance HIPAA Compliance, add Security and Compliance WAF and Firewall, and host on Hosting and Infrastructure Plans.
